I am able to upload documents into the actual form? I am creating test and what participants to be able to review the material (open and download to their devices) prior to taking test.

If it's a PDF document, you can do so by using the PDF Embedder widget as it allows you to upload and embed PDF documents directly in your form.
Here's a guide on how-to-display-a-pdf-document-on-your-form.
