All options are correctly checked but we still don't receive a PDF attachment to completed forms...

Is this the form https://form.jotform.com/211246122601035 in question? I just cloned it and the PDF attachment is working as expected.
Could you please try recreating the email notification and enable the PDF attachment again? If the issue still persists, please let us know.
