I did check your form and found that the signature field of the "Consent to videotape" section is not added in the original PDF. That is the reason why the signature is not being displayed in PDF.
I have gone ahead and added the signature field and date field to PDF. Please check the screenshot below:
It seems there are several fields of your form which is not added in the correct location of the original PDF. Please check the screenshot below:
You need to move the text boxes to the desired location of the original PDF.

Hi. I'm still having the same problem. The signature for the last page (Videoconsent form, attached in email thread above) is not aligned with the. PDF. I tried doing what you suggested but it's not working. Also, I tried the fill in the blank function but it's not working either. Could you remedy this for me? I followed all the prompts you gave but it's still not working.

Also, in trying to edit it, the page and font became half the size. Can you fix that too? And is there a webinar to learn how to edit my documents, rather than asking you each time? Thank you!

I have now moved the signature field to the correct position in the Original PDF.
For fill in the blanks, can you please let us know what the label of the field is and where you want that to show in the PDF?
