Submission emails does not show the response.

    I recently created a form and when I get the response via email is VERY messed up.

    So for example look at the latest submission in JotForm and then view what it is I receive (see below) PLUS the name (required box) does not come thru!! This is CRITICAL to us...

    How often have you eaten at the KMCC Sports Lounge?
    What do you think of the food selection currently offered by the KMCC Sports Lounge? test1
    How would you rate the value of food to the price?
    If you could add to the menu what particular food items would you like to see on the menu?
    How long are you willing to wait for food?
    If the Sports Lounge is changed into a European Pub would you like to see a German Style Bar?
    Which is more important for a European Style Pub, the drink selection or food selection? Used to  attend GBY some time ago
    What type of food would you like to see in a European Style Pub?
    What type of atmosphere would you like to see in a European Style Pub?
    What other suggestions would you have for a European style Pub?
    If the KMCC Sports Lounge was converted into a European Style Pub would you be more likely to patronize the facility?

    I looked on your account, but I see you have quite a few forms so I am unsure of the form in question. Can you please tell us which form are you having this problem on?

    Hello Sandy2121,

    Have you cloned your form and then changed the form fields? 

    I see that your notification email template has question which does not exists in your form. You must have cloned the form and then edited the form fields.

    Please be noted that when you edit the notification email template, any changes your make in your form will not reflect in the notification email template. You will have to manually update your email template as well.

    The most convenient way to solve this problem is to delete your notification and add a new one. When you add a new notification alert, you will see that all question of your form will match with the notification fields.

    I didnt edit.
    I did this form like I have done any others.
    I choose the form and then put my wording - naturally- in it
    I'll try another form when i get a chance

    Hello Sandy2121,

    Please delete the notification email alert you have in your form and add a new one. That should solve your issue. The following guides should help you:
