Why my form is not loading mobile responsive?

Same here not responsive nor optimized on mobile i am using samsung galaxy s6

I have checked the forms in your account and I can see they load responsively in an Samsung Galaxy s6 device, I have also tested the form in other devices with Android 6 and I can see the forms load fine.

Have you embedded your forms in a website? 

May be that this happens when you embed them, please do note that the style applied to the webpage where the form is embedded may also affect how it looks in a mobile device.
