Why my form get stuck on submit?

Hey! Love your site, the forms are fantastic, but I am having an issue where the form on my wordpress (Divi theme) hangs when I attempt to submit it.

Both instances of the same code seem to have the problem, so I feel like I may have done something incorrectly on the backend but just can't figure it out.
