Clear up confusion on consent forms for pharmacy

    Question 1 I'm struggling to decide how best to get consent forms.

    I'm using a JotForm for customers to schedule Covid Vaccines. I'd like for them to complete an ONLINE consent form that gets submitted to me (my current form has a PDF attached that customer's are struggling to print and complete).

    Can you advise me of the easiest way to do this....So that customers are not confused about what to do? Currently, I'm trying to email a link to them with an online consent form after they schedule an appointment. I'd like to automate that step.

    Question 2 For patients that do not have ability to self-schedule online, I'm scheduling them manually. What is the easiest way to block out those appointment times on my JotForm so that the other customers don't double book?

    Good day, thank you for contacting us.

    Q1: You can simply enable the PDF attachment of your Autoresponder, so when they fill and submit, they will get the PDF submission attached via email:

    Q2: You would need to fill and submit the form for them, you just need to preview the form:

    The appointment slot that you select will be blocked after you submit the form.
