Why was my form disabled?

Yesterday we recieved notification that our account was almost at its limit for submissions, and that the forms would be disabled today. However, we completed a few of the requirements for obtaining additional submissions and my account currently says we have 188 of 200 submissions for this month.

Why were the forms disabled when we increased our monthly quota before we reached our max? Please advise.

I checked AudiLists account and found a premium account, that is far from its limits. Can you please provide us the account username or the form's url so we can look better into this question?
