Word "undefined" appearing at the top of all my forms

I have made a few forms over the last two weeks or so, and I went to make a new form today, and just noticed that when I preview it, the word "undefined" is at the very top of the form.  I went back to check forms that I had previously made, and it is appearing at the top of all of them.  How do I get this removed, as the word is appearing when its embeded into my website as well.
