I have 2 conditions that are not working and they are set up correctly.

Form is located here.  When you sort the conditions by "Rule Field" the second two conditions do not work.  They are set up like the first one listed (which works) but the second and third do not.  Please help.  Thanks!

I have checked the last two conditions on the 'Rule field', but now I realize that you are referring to some other conditions.
