Upon checking the Recipient E-mail of your notification, I found that it was in our bounce list:
I have removed it from there, so you should be receiving your notifications now. Just to let you know I checked your other form, I noticed that the Sender Name field is empty:
Sender Name :Make sure you assign it to a field from your form where the user enters her name (usually the"Name" field -or similar)
