Form Theme has disappeared

Hi - the "theme" I selected for my form has disappeared and the theme options don't appear to be "live" at the moment. Is this temporary or is that option no longer available?

Based on your last form ID: http://www.jotform.us/form/40708805501146. You are using the Jot Theme which is present to the form. Are you referring to a different form?

You should be able to choose a theme from the Form Styles - Theme section. Please follow this guide: http://www.jotform.com/help/184-How-to-Change-Form-Themes.

It has reappeared after I rebooted but was definitely gone at the time I was working on it earlier today.
