Form no longer displays on HTTPS website in Chrome

I have multiple forms but only the form on an https website is blank - and this may have started when the HTTPS was launched.

Could this be the problem?

Have you try to set your form by clicking the "Secure form checkbox" like the screenshot below? if not, please try this one and I think that's the problem why you can't view your form when you used the HTTPS, then embed the secure form and replace the old one.
