contratar o plano Premium

ola parabéns pelo sistema! minha questão é, se eu contratar o plano Premium que me permite usar até 3 sub user account, eu posso usar cada formulário em um site diferente e com emails diferentes, ou só posso usar o mesmo e-mail?

Even though you don't create a sub-user account, you can use 1 or more forms to different websites repeatedly. Regarding the Premium plan, if you upgrade to a higher plan, you'll be able to increase your limits in regular submissions, SSL and payment receives, total Upload space storage. You can find more details of it here: http://www.jotform.com/pricing

I am sorry, but I am not sure if I understood you correctly.

I presume that you are asking if:

1. you can have any number of forms in your account and
2. if you can embed any of them multiple times

The answer to both would be yes. You can have any number of forms and you can embed each of them as many times and on as many pages as you want.

Now you also mention subscription. Do note that if you purchase some plan, the plan is not purchased per form, but per account, meaning that all of the forms share the allowed quota assigned to your account as shown on our pricing page.

I also presume that you ask what happens if you reach the quota. If so, the answer would be that you will get an email notification that you are near your quota before you actually get to it, allowing you to take possible actions such as upgrade or free up space before it gets over the quota.
