We are not receiving notifications for submissions on "Rate Request from NHAtravel.com"

Hi,
That is because your notification email settings are not properly configured. Your first notifier doesn't have the recipient email address and the sender name is not set:
And your second notifier also doesn't have the sender name and email:
You will not receive the form email if any of these settings are missing or not configured to your form notifiers. Kindly check and correct it.
