    Dear Jotform,

Your system suspended my account by mistake (may be).  Please check my account and reactivate my account to publish my forms.

    Thanks

    Hello,

I did check your account and found that it is already active. Are you still not able to sign in to your account?

Upon taking a closer look, I found you have one another account which is currently suspended. One of my colleague have already responded to you on the following thread:  http://www.jotform.com/answers/507583-My-IP-Was-Blocked-Can-You-Unblock-My-IP#4

    Thank you!

Thank you for clarifying the issue more.

I found that your account was actually suspended because of two jotforms for "Alolika" and "Arpita" you have which appeared to be suspicious of phishing or obtaining sensitive information which would be a violation of our Terms of Usage. This is the reason why your jotforms are disabled.

Can you please explain the purpose of these two jotforms and what they are being used for?

Also can you please provide us with a link to the webpage or site that you are using them on if any?

Thank you KadeJM for asking me the purpose of the forms.

Actually this "Alolika" ar "arpita" are the name of my students and you can check the form name submission which is also allotted for a particular student. Only "submission" form has submit facility and others forms have not. other two are only for accessing the link (Study materials/Class note/Assignments) providing by me.

I have also another form where when student provides his/her student Id and submit the form, he/she redirect to their allotted page (here I have use your thank you redirect facility).

I hope I can explain you the purpose properly. It is actually used for educational purpose.

    Thank you.

    Hi,

I believe one of my colleagues is looking into your situation in this thread:

    http://www.jotform.com/answers/507583-My-IP-Was-Blocked-Can-You-Unblock-My-IP#4

It appears as though the IP block itself has been removed though.
