Get Referrer widget is causing the form design to get broken in iPad 8

The get reffer widget we have implemented but the form on ipad 8 are broken. Get refferer helps us to detect where from a order was made but we need properly working embeded form

Is there any way you can send us a screenshot of how the form appeared to your iPad 8 device? I'm using an emulator and this is how it appears to my end:

The issue, as you could see it, is the form field inputs which overlaps the right side of the page. Is that the problem?
