I need step by step guide how to use JotForm

Personally, I need a step by step approach. 
First do this here, then do this, etc.  And the work area is small and confusing.

The form I require is straightforward and basic and yet, I am unable to take even the first step. I need to accomplish this - http://www.envirosnowmelt.com/creditcardorders.html 

Paid, phone or not. The forms that were available in programs were far more straightforward than this. You entered text, clicked to have a label, clicked another for a field to enter . . .

Here are the steps needed to add fields to your form:

First, create a new form in the form builder:

Second, click on the fields you would like to add to your from the options on the left:

If you need to collect their name, click on the "Full Name" field.  If you need to collect and email address, click on the "Email" field, an address, click on the address field:

Next, to change labels for your fields, double click on the label and change the text:

If you need to add products, Add a Payment field with for your preferred payment processor:

This will help you setup your products as well.

Since your form is rather lengthy, it would take me quite a while to type out step by step how to create it but this information should at least get you started on the first steps to making your form.
