I think right now the form is aligned to the centre. But I would like it so it's completely to the left. (so the boxes are in line with the text)

You are referring to this space right? You want to fully move the form towards left so that it will be aligned to the text.
Please try to inject this custom CSS code.
.form-line { padding-left: 0px !important; }

Thank you, I have now changed it :)
There is only one problem now, it seems there is some cut off text. (What are some of the little things your kids or spouse does that you absolutely love and want to remember?) How can I fix this?

Please try to inject this custom CSS code in your form and see if it fix the text that are being cut off.
ul.form-section.page-section {
width: 75%;
}
