Could you please give us the link to the page on your website where your form embedding code is added to?
That will allow us to check it out and see what is the reason why your form is not showing up and help you set it all up. :)

Also the submissions are not coming through right and I can't setup my notification email for when someone submits and the responder emails are not working at all emailsPlease note all the problems I am having is on the Jot Form website, please fix this I am a premium customer and I run all my services and products for my company through JotForm.

Thank you for the additional details.
This is strange and we will check it out and help you resolve it. What I think that is happening is the browser cache.
To verify and if it is, resolve this, you should follow these steps: 15 Ways to Clear Your Browser's Cache - wikiHow
Now I have moved your second paragraph about the email notifications to a new thread since it is a different issue, so we will help you resolve it as well on the following thread: http://www.jotform.com/answers/614505
Now, just so that we can inspect this issue further (with the empty direct link), can you tell us what is the form that you are having this issue with - if the steps on clearing your browser's cache do not work.
