Why won't my forms show up on my website?

    It appears that both of my forms - Request a Quote and Make a Payment - will not populate on my website.  I repeatedly get a message that says "This form has been disabled."

    It appears that your form is asking for credit card details using plain text fields. Our enhanced system phishing filter detects the form and auto suspend your account. Please be informed that we do not allow this type of forms in our system. We consider it as Phishing which is against our Terms of Use (https://www.jotform.com/terms/).

    I see that you are doing a travel booking business that requires this information from your customer and I can see that it's not intended for phishing. But unfortunately, this approach is not safe for your clients and our system is as risk if we continue to allow this type of forms. That is why we've enhanced our filter to detect credit card fields in a form that is not integrated to our Payment forms.

    In this case, we always recommend integrating our payment tools to the form and you can do authorization only so you can capture the amount later as agreed. Here's our guide for that:

    If you do not want to predefine the amount, you can select the "Donation" and uncheck "This is a donation. We are a non-profit organization." so the payment will still come as a product/service:

    I have reactivated your account, but I have removed the credit card fields from the form.
