Form not displaying properly on Dreamweaver preview

I put the code on Dreamweaber and just apper this

and when i try to preview on any platform Just charge some parts form form I think its to heavy to charge.

What can i do?

Please publish it and preview it on an actual browser so the form resources like stylsheets and scripts will load properly.
