My forms are not working

I checked your form and noticed that you have a form named Login Reset - http://www.jotform.us/form/53117170908150. We do not allow jotform to be used as a login page. However, I checked your form and it does not have a field that is against our terms of use. Please try to rename the form to avoid being suspended again. I also checked one of your form - http://www.jotform.us/form/53116470179152 and I noticed that you are collecting Social Security Numbers which is against our terms of use. I have activated again your account. Please change the form as suggested above to avoid to your account to be suspended again.
