why are my forms disabled?

I tried to embed the forms into my website and they don't show up.

I then used just the link to the jotform you provided and it says form disabled.  Why?  How do I fix this?

I've checked your account and one of your form is disabled due to phishing, I can see you are asking for Credit Card information, that is not possible, I would suggest you to use some of this features if you are requesting for CC info :
