I have not received a response about this issue yet. The submit button is on the form in jotform but once I open the form on my website, the submit button is no longer there. I have put a link to the page below.

I would recommend you to embed your form with the Iframe code: http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code
Before you embed the code, paste it in a text editor like WordPad, you will notice two part of codes, only copy and embed the Iframe part in your webpage, example:
Make sure to set the proper height in the iframe code to display all your form.

I would suggest you to please use the following iFrame embed code and see if that solves your problem:

Thank you - This is better except now there is a large amount of space below the submit button and the text at the very top (this questionnaire is designed to...) is cut off on the right side. Any idea how to fix that?

I did check your webpage where you have embedded your form and it does not display any blank space below the submit button. Please check the screenshot below:
Increasing the width of your iframe will actually solve the issue where the texts is getting cut. Please use the following iFrame embed code and that should helps you:

I am having a similar problem with two of my other forms. The right side of the below form is cut off - can you help me with this?
