PDF Form: Layout issues on generated form

    Oy, vey!  I used the pdf converter and it looks just horrible!  Everything is out of alignment and the send button is not functioning.

    Please halp!

    Could you please provide us with a form name in question?

    The submit button should work in PDF readers like Adobe Reader and Foxit Reader.
