Additionally, the Date and Time are not showing up when the form is received to a gmail account (email@example.com test email account).

I checked your email and names of date and time fields are different than the actual names. I'll illustrate:
This is your email:
And this is how your fields are named:
So, just change the names in your email so they will match the actual names and everything will be fine. If you need any assistance please let us know.
