I need a contact form that will generate information on a retail account and also on a wholesale account to see different pricing

My understanding is that you want to have a form wherein the user can choose whether it is a retail account or a wholesale account.

I appreciate your prompt response. What I'm looking for would be located in
the contact section of a website. In this section the visitor would fill
out the form with such information as email address, name, address etc etc.
The commercial visitor would have the same type of form but would indicate
that they're commercial. The reason for this is that my client wants the
commercial prices that would be available only to the decorators,
designers, etc.

Did you check this demo form (https://form.jotform.com/63365263684968)? This demo form has a Show and Hide Fields conditions on it. It means that there are certain elements that will appear based on the selected option. For example, if the user chooses "Retail", then the Retail section will appear.

You may also create a second form for the Commercial visitors if you like.
