My understanding is that you want to have a form wherein the user can choose whether it is a retail account or a wholesale account.

Please check this demo form: https://form.jotform.com/63365263684968. You can clone it if you like.

If this is the type of form you want, let me know so that I can provide a step by step instructions on how to do this.

If not, please provide more information so that we can assist you properly. Thank you.

    Hi Jan,
I appreciate your prompt response. What I'm looking for would be located in
the contact section of a website. In this section the visitor would fill
out the form with such information as email address, name, address etc etc.
The commercial visitor would have the same type of form but would indicate
that they're commercial. The reason for this is that my client wants the
commercial prices that would be available only to the decorators,
    designers, etc.
    Thanks,
    Abe

Did you check this demo form (https://form.jotform.com/63365263684968)? This demo form has a Show and Hide Fields conditions on it. It means that there are certain elements that will appear based on the selected option. For example, if the user chooses "Retail", then the Retail section will appear.

You may also create a second form for the Commercial visitors if you like.

    Hope that helps. Thank you.
