Do you have a way to upload documents by pulling them directly from Box?

I would like to create a 'Production Request' form. This should allow people to select from documents already in Box and add them to their form submission. Ideally, I would like to have selections per document. Is this possible? Something like the attached screenshot but with a section to upload the document straight from Box.
