On my created form, when I click on submit form my brower times out and no result or submission exists. I have tried different browsers and re-created the submit button. In preview and normal mode it times out without collecting the result. I am sure I am doing something incorrect, could you help me out?Page URL:

Could you please try again? I just tested this form and the submit button works for me. I tried many scenarios and they all work. Here's an example screenshot:

I have tested your form, but I was not able to replicate mentioned issue. I successfully submitted your form and got redirected to Thank You page.
Are you still unable to submit your form? Maybe it was temporarily issue or you had an issue with your internet connection. In case that you have issues with your embedded form, please provide us URL of your webpage.

So I am still having issues with the form but don't really know what to do about it. If I access the form from outside our University network everything works great and it submits. It will not submit if I am connected to the University wifi or ethernet, unless I change the DNS server to googles, or opndns. The problem I am having is that our students would be filling out the form while connected via our wi-fi. It would be a hassle and not feasible to have every student change their dns settings just for this. Anything that could be done?

You would need to ask the school's IT department to check the network's Firewall and allow access to jotform.com. Jotform is blocked in the network and this is the only reason since you're able to submit it outside. Unfortunately, we can't do something about it since we do not have the control over the network settings.
