You can change the form page title in the Advanced Preferences settings. Here's how:

    1. Open your form and click the Preferences button in the Form Builder.

    2. In the Preferences, select the Advanced tab. After that, click the Show More button and Edit the Page Title. When you're done, click the Save Changes button.

Hope this helps. If you need further assistance, please let us know. Thank you.
