Basically, can Jotform alllow for us to have a form on our website where a user can upload an image and a caption and when it is submitted, goes through a moderation process. Once approved, it would appear in a gallery? Basiclally the first page would be the form and the second page would be the gallery.

You can make a form to upload any kind of files.
However, your scenario would involve a programming task.

Another option that you can use is configure your upload form to send images to your server's FTP folder. Then, handle these uploaded images with some custom gallery application or use some existing solution like Coppermine Photo Gallery. I think that Coppermine Photo Gallery has Upload Approval feature so this is probably something you are looking for.
