My forms are disabled.

I created a form for a game me and some friends are playing but it says my account has been suspended.

The Phishing detector was being triggered by your form https://www.jotform.us/form/63458272588166 due to the password/passcode field you had used on the form. The form was being flagged as phishing form and violates the Terms of Use.

I suggest you modify your form and correct those fields. You can delete them or just rename them to something else other than password or passcode so that it will not be mistaken by the Phishing detector.
