I created a clone of a form from this account to by used and personalized by another account. The cloning and download/customization was successful, but the form on the secondary account has been disabled. How can I fix this?Page URL:

Can you please confirm that this was the other account form https://www.jotform.us/form/63458266412155 ?
I just checked it and I found the username is active and the form is available at this time.
Let us know if issue still persist.
