How to create a form with repeating groups of fields?

I'd like the user to complete some standard fields and then indicate how many dogs they have.   Then, to present them with a repeating group of field - repeated by the number of time they entered.

Example:

How many dog do you have - 3

Name Age Breed

Name Age Breed

Name Age Breed
