I think you could use a drop down to show the providers, if they are multiple, this way you will list all of them so the submitter chooses which will be used:
Now, if there are appointments then the best option will be to display them using a widget, here is one that may help you:

It will also allow you to set the available number of times that an option can be selected, if payments will be processed in the form then a payment integration will need to be added, here you will find them: https://apps.jotform.com/category/payment_processing
