I understand how to make a field required.
However, we're struggling with a conditional requirement.
Details:
Form: Health History Form
The Question on the form is, "Do you have Dental Insurance?"
Answer Options: Yes No
***If they answer yes, we'd like to make the subsequent insurance fields required, ie, policy number, address, phone, etc.
If they answer "NO" to having dental insurance, we don't want those fields required because they'd have nothing to fill out.
Please advise.
Thank you,
Christian

Christian,
You can use the form Conditions Logic feature to achieve this.
User guide: How-to-Make-a-Field-Required-Based-on-User-s-Answer
Here is an example form https://form.jotform.com/63476994411970
Here's how I created the test form.
Hope this help. Let us know if you need further assistance.
