Can you make certain fields required only if they check yes

    I understand how to make a field required.

    However, we're struggling with a conditional requirement.

    Details:

    Form: Health History Form

    The Question on the form is, "Do you have Dental Insurance?"

    Answer Options: Yes    No

    ***If they answer yes, we'd like to make the subsequent insurance fields required, ie, policy number, address, phone, etc.

    If they answer "NO" to having dental insurance, we don't want those fields required because they'd have nothing to fill out.

    Please advise.

    You can use the form Conditions Logic feature to achieve this.
