Our account is suspended?

    Noticed we were no longer getting submissions, and when I tried logging in to see what the problem was, it wouldn't let me. Instead, there was an improperly formatted (raw HTML) pop-up window that said our account was suspended due to TOS violations. How could this be possible? We've been using these forms for years with no problem. If we are doing something wrong, we would be happy to fix it... if we knew what it was we actually did. Need to fix this ASAP. Thanks!

    Our sincerest apologies for the inconveniences caused. 

    Your account was mistakenly suspended probably due to some confusions on the phishing rate of your forms. We have anti-phishing tool that checks every forms if they fall off in the phishing category. We have reinstated your account after finding no violations of our terms.

    Your forms should now be working normally as usual.

    Thank you! Everything seems to be back in order. Any idea what it was that set it off? Anything we should change?

    Seems like the phishing system flagged your forms as possible phishing by mistake. I have reviewed them, and they are okay. We have white-listed your forms to prevent this from happening again.
