i am unable to edit form properly

I am in Request for transportation 2016-17 and when i click to edit the form is not coming up correctly.  it is coming up as if i am filling out the form from our website not editing it.

Could you please let us know more about this problem when you're in Submissions page and click Edit, a form should open (prefilled with data for that particular submission), is that happening to you or is it something else going on?
