Name of form does not show up on browser tab.

    The name of my form is “Client Information.”  However when I preview it the tab on my browser says “Quiz.”  How do I get the tab to say “Client Information”?

    Hello,

    I have checked your form on Google Chrome, Mozilla Firefox, and Safari Web Browsers. The tab name of your form appears as "Client Information" on all of them as you can see the screenshot below:

    I recommend you to clear your browser caches and try again.

    Besides, I have checked your form and I have seen that you receive Social Security Number in your form as the following screenshot below:

    You are not allowed to receive Social Security Number from your users in your form due to our terms of use.

    So that, I have removed that forbidden field from your form.
