Phising: Account suspended

my acccount has been suspended but I dont had nothing...

I have checked your account and forms. This form contains a "Password" field. You can not directly get personal sensitive information such as password, credit card details. Because it violates our terms of use. If you insist on collecting any of this information, your account will be suspended again.  

I have deleted these fields and re-activated your account. Your forms are enabled now.
