account suspended

Hi,this is my own account. I use it for my business, it's not spam account. Please help me to restore my account. It is very necessary for me. Thanks!

The account was automatically suspended by our Phishing detector system as it is asking for "facebook" and this is a keyword that is considered suspicious, after checking further I can see the form is not requesting for the facebook login credentials which would violate our terms of use.
