why does the submit button dissappear once i start to put information into contact fourm

everything shows up from the beginning but once i start to put information into the contact forum on the website the submit button disappears and i am unable to submit the whole forum.

I checked the form at http://www.jotform.us/form/62786228263160 and did not find any issue.

There is possibility that you are using the form in IFrame and the height of the Iframe is not long enough to accomodate the form.
