How can I add a hyperlink to my form?

According to my understanding, you are wondering how to add a hyperlink in your form.

#1- Add Text form field to your form. 

#2- Click Edit HTML.

#3- Click Insert/Edit link icon in this coming page.

#4- Enter your URL here.

Target also means that at which window your form will be opened.

After you set up your hyperlink settings click OK.

#5- Click Complete.

#6- This is the final view.
