I am not quite sure, but I think what you want to do is create a form which looks like that paper form in your attached image.
If that is what exactly you want to do, I suggest that you use the Matrix tool to create your form.
You can find the matrix tool under "Survey Tools":
By default a matrix field has radio buttons selected as the input type.
You can change this by clicking on the Input Type button then choosing the input type you prefer (in your case, text box):
Click the Rows and Columns button to edit the labels for the rows and columns
To adjust the column widths, modify the values in the custom CSS code I injected on the said form. Here's how to go to the custom CSS area: https://www.jotform.com/help/117-How-to-Inject-Custom-CSS-Codes.
