When I "preview" a form that I created, some questions are not there. Why?

I am currently working on a few forms and each time I "preview" the form, it doesn't show the entire form--some questions/items are missing.

I'm assuming you're referring to this field: 

Are you available to work 40 hours/week, Monday-Friday, starting June 13th and ending on July 21st? As well as staff training days beforehand on June 3rd, 10th and 12th?

The reason it's not showing up on your form preview was because of this condition:

So, if you select Indianapolis for the field Please select the TeenWorks city in which you are applying for then that's the only time the field will show up:
