My form is disabled.

My form on https://form.jotform.com/63564984261162 is disabled, and cannot work. How do I fix this problem?

I have checked and your account is autosuspened by our phishing system. Inside of your form Christmas Break- School Site Login, you are asking for user login credentials (username) and that is in violation of JotForm Terms of Use.

Phishing - You agree that Company may terminate your JotForm Service immediately If a form is created or JotForm is found to be used to: Obtain credit cards sensitive information, get social security numbers, show content with child pornography. Or if a form was made to steal user login credentials. All these activities are considered as Phishing and any account along with the guilty forms will be suspended immediately. 

I also saw that inside trash folder you have three login forms. 

I have deleted username field from your form Christmas Break- School Site Login. I would suggest you to change the title of that form and remove login word. 

I have also purged login forms from trash folder. 

I have activated your account and your form is now working. Please do not create forms that are against our terms of use or your account will be suspended again and we will not be able to activate it.
