Driving Distance widget

I have used this before with no issues, but this time I cannot get it to work poperly, can you please check this for me. Regards David

Where are you getting that result? I have tested your form and Driving Distance widget is working properly.

I have also cloned your form and tested my cloned form. Data is shown properly inside of email Notification and on submission page. 

Can you please test your form again. If the issue still persist, please provide us more details about it. Also if possible please provide us a screenshot. 

If you have issues with some other form, please provide us ID of that form. In case that your form is embedded provide us URL of your webpage.
