Why is my form disabled?

    Hello Jotform team!


    I just created my first form, a subcontractor application for my construction company. Although my preferences show it is enabled, when I try to access it, it shows an error:disabled box. 


    Any help on getting this resolved and the form active? I need to embed it in my website, and time is of the essence.

    Hi Ana, thank you for contacting us.

    Unfortunately your account has been autosuspended because of a form that violates our terms of use, by the name "Subcontractor Application".

    This form is asking for passwords, and as a result it was detected by our systems as suspicious.

    Please consider adapting the form to comply with our terms of use, especially sections "Phishing" and "Collecting Sensitive Information" and we will be happy to reactivate your account after examination.
