I used your template for the Tell-A-Friend to create an invitation to church. It works well except for sending to the friend and the person who sent it. It only sends to the form owner. Thank you for any help.

In addition to what my colleague has stated, I noticed that your form has no autoresponders. It needs two of them: One that goes to the user who submits the form and the other to the person they refer to your site.

Just to clarify, for the autoresponder that's intended for the user who submits the form, select From: in the Recipient E-mail field of the Sender and Recipient Details section, as shown below:
For the other autoresponder (that sends the message to the referral), select To: in that same field.
